2026/27 Registration Form

This form must be completed annually by the Parent or Legal Guardian.


Please complete the information below and submit payment for the annual membership fee, selecting either the full payment option or the instalment plan.


If you choose the instalment plan, the first payment will cover membership from the date of payment until December 2026. A second instalment must be paid before training commences in January 2027, covering the period January to May 2027.


Parents or Legal Guardians are responsible for notifying the club promptly of any changes to personal circumstances, emergency contact information, or medical details throughout the membership period.

Age Group / Team

Age groups are based on a player's age on 31 August, in line with grassroots football regulations. This ensures children play with others of a similar age and stage of development. Please note that our U11 players currently play up in the U12 age group If you have any questions or concerns, please speak to your coach.

It is important that you fill in the rest of this form as fully as possible. Failure to tell us information could mean that the Health, Safety & Welfare of your child is compromised. The club cannot be held responsible if the information has not been shared and updated.

For safety reasons, players who require glasses may not be permitted to train or play unless they are wearing suitable sports eyewear approved for sporting activities.

Images and Videos

At times the club may wish to take photos or videos of the team or individuals in it. We adhere to the FA Guidelines to ensure these are safe and respectful and used solely for the purposes they are intended for. Please indicate if this is acceptable?

Non-Football Activities

There may be occasions when your child is involved in planned and structured non-football activities such as a trip to the cinema, bowling or participating in a fundraising event. The club will have collected appropriate information in preparation for the activity (e.g. about the journey, the children’s various needs and helper skills), identified any potential risks or dangers, minimised the risks and dangers by careful planning and precautions and made sure we know who is responsible for putting precautions into place (e.g. who will carry the First aid kit). This process is called a risk assessment.

Code of Conduct

As an England Football Accredited Club, we are committed to providing a safe, enjoyable, and respectful environment where players, parents/carers, coaches, and volunteers can develop and enjoy football. By signing this consent form, you acknowledge and agree to the following standards of behaviour.


PLAYER

  • Show respect to others.

  • Play by the rules.

  • Play fairly and to the best of my ability.

  • Shake hands with opponents and match officials at the end of each game.

  • Listen and respond positively to my coach or manager.

  • Speak to someone I trust within the club if I am worried or unhappy about anything.


PARENT / CARER

  • Remember that children play football for fun.

  • Applaud effort and good play, as well as success.

  • Avoid criticising mistakes.

  • Respect the decisions of match officials and encourage players to respect opponents, referees, and officials.

  • Remain outside the field of play unless invited by a coach or official.

  • Never engage in, or tolerate, offensive, insulting, discriminatory, or abusive language or behaviour.


MANAGER / COACH

  • Show respect to others.

  • Adhere to the Laws of the Game and the spirit of football.

  • Promote fair play and high standards of behaviour.

  • Respect the decisions of match officials.

  • Never enter the field of play without the referee's permission, except where player welfare requires immediate action.

  • Never engage in, or tolerate, offensive, insulting, discriminatory, or abusive language or behaviour.


By completing this registration, I confirm that I have read, understood, and agree to uphold these standards, and will support my child in doing the same. I also agree to help the club maintain a safe, positive, and respectful environment for all participants.

When information is no longer required, it will be securely deleted from our systems.
