2025/26 Registration Form

This form is to be completed by the Parent or Legal Guardian annually. Please complete the form below and make your payment for the annual membership fee of £135 or the 1st instalment of £70 to cover from now till December 2025 (this includes all training fees) and then we will require payment of £70 payable prior to the start of training in January 2026 for January – May 2026.


Parents or Legal Guardians are responsible for informing the club of any changes to circumstances or medical details as they occur.

Images and Videos

At times the club may wish to take photos or videos of the team or individuals in it. We adhere to the FA Guidelines to ensure these are safe and respectful and used solely for the purposes they are intended for. Please indicate if this is acceptable?

Non-Football Activities

There may be occasions when your child is involved in planned and structured non-football activities such as a trip to the cinema, bowling or participating in a fundraising event. The club will have collected appropriate information in preparation for the activity (e.g. about the journey, the children’s various needs and helper skills), identified any potential risks or dangers, minimised the risks and dangers by careful planning and precautions and made sure we know who is responsible for putting precautions into place (e.g. who will carry the First aid kit). This process is called a risk assessment.

As an FA Chartered Standard Club it is essential we recognise and adhere to the clear guidelines that are in place to ensure the safe, happy and respectful environment where our players, parents & volunteers can develop and enjoy their football. Please read the following statements and when signing this consent form you are also agreeing to these guidelines.

  • Show respect to others

  • Shake hands with the other team & referee at end of game

  • Listen & respond to what my manager/coach tells me.   

  • Talk to someone I trust in the club if I am unhappy about anything


PARENT / CARER

  • Remember that children play for FUN

  • Applaud effort & good play as well as success.

  • Avoid criticising mistakes

  • ALWAYS respect the match official’s decision & encourage players to respect their opponents, referee & match officials

  • Remain outside the field of play

  • Never engage in, or tolerate, offensive, insulting, or abusive language or behaviour


MANAGER / COACH

  • Show respect to others

  • Adhere to laws & spirit of the game

  • Promote fair play & high standards of behaviour

  • ALWAYS respect the match official’s decision

  • Never enter the field of play without the referee’s permission

Privacy Policy

Your privacy is very important to us. Accordingly, we have developed this Policy in order for you to understand how we collect, use, communicate and disclose and make use of personal information. The following outlines our privacy policy and practices relating to the management of personal information.


This policy applies to registration, subscriptions and notifications for Mere Town Youth F.C.

Who we are?

We are Mere Town Youth F.C. and our data protection officer is Corrie Tatum - meretownyouthfc@gmail.com


What information do we collect?

We will collect names, addresses, contact details, and medical & religious information for both parents and children registering to sign up to play for Mere Town Youth FC.

We will collect this information upon registration to Mere Town Youth FC.       

The data is collected via our website which a cloud-based application. We will share the information with Dorest Youth Football League and The Football Association Limited via The Whole Game application which is also cloud-based should your child be selected to sign up to the squad to play league fixtures. Your information will be used for registration, subscription, the latest news, and marketing. We never share your information with any other third parties. Both cloud-based applications use passwords for protection and your information is stored securely within the cloud.


What legal basis do we have for processing your personal data?


We require your consent in order to proceed with the registration, subscription, and marketing (which we believe to be in your interest and will be kept to a minimum.) 

If you wish to unsubscribe at any time you can email -meretownyouthfc@gmail.com at any time and we will immediately delete all your records from our storage. Please note that this could result in your child being unable to continue to play and train for Mere Town Youth F.C.

Only assigned managers and club committee members have access to your personal data. Each member of Mere Town Youth FC that has access has agreed to protect the information.


How long do we keep your personal data?


We will hold your information for one complete year unless registering for the following year. Upon departure or unsubscribing your details will be deleted immediately. 

The information upon deletion will be erased from the cloud storage.


Your rights include:

• asking for access to personal information

• correction and deletion

• withdrawal of consent (if processing data on the condition of consent)

•restriction of processing and objection

• lodging a complaint with the Information Commissioner’s Office
